Vaginal estradiol loading dose

The concept of a vaginal estrogen loading dose is not grounded in robust scientific evidence; rather, it is a convention based on labeling and a manufacturer’s dogma.

Whenever I prescribe vaginal estrogen, I do not include the loading dose.

The loading dose typically consists of daily administration for two weeks, followed by maintenance dosing twice weekly indefinitely.

There are two primary reasons for my approach. First, the regimen can be confusing for patients, often resulting in follow-up calls for clarification regarding the transition after two weeks. Was that three weeks? How many times a week now? Second, in individuals with atrophic tissue, increased absorption during the loading phase can elevate systemic estradiol levels, potentially leading to side effects such as breast tenderness or headache. This may undermine patient trust, particularly when they have been assured that the treatment is local.
